Applicants must submit the following documents
Renewal of Passport/Damaged Passport (Adults):
- Completed passport application form, click here to get form
- Two (2) identical full face non digitized colour photographs with a dull matt finish with ears clearly visible. Minor's photograph must be certified by a recognized official
- Expired Passport
- Original Birth Certificate
- Original Marriage Certificate, where applicable
- Original Death Certificate, where applicable
- Original Adoption Register, where applicable
- Divorce Judgement, where applicable
If born abroad, in addition to the Birth Certificate, please submit one of the following:
- Certificate of Naturalization
- Letter of Certificate of Citizenship
- Certificate of Registration

- Legal guardianship document
- Minors must be accompanied by parent(s) or legal guardian.
